Insurance Reimbursement for 3D Areola/Nipple Tattoo
Overview:
Reimbursement for 3D areola/nipple tattooing through your insurance provider is common, though not guaranteed. To improve your chances of reimbursement, we encourage you to request a predetermination of benefits from your insurer prior to your appointment. This digital form will generate two PDFs to support your predetermination request: a template letter of medical necessity for your physician’s signature and a predetermination of benefits request letter. These documents will be sent directly to your email. We are not in-network with any health insurance providers and do not bill or communicate with insurance companies on your behalf. Full payment is due at the time of service. After your appointment, we will provide a superbill (detailed invoice) that you may submit to your insurance provider for potential reimbursement. Please follow the steps below for obtaining predetermintaion from your insurance:
Step 1 - Contact us to Schedule a Phone Consultation
Please call, text, or join our waitlist to schedule a phone consultation. This allows us to confirm that we can meet your expectations, that you are outside any required healing timeframe, and to clearly explain the process. It also gives you the opportunity to ask any questions about the service, payment, insurance, or next steps.
Step 2 - Gather Required Documentation

Request your physician complete the "Letter of Medical Necessity" (provided at link above)
Step 3 - Contact Your Insurance Company
Call the member services number on your insurance card
Ask the representative: 1. Does my plan cover CPT 11921 by an out of network provider? 2. What documents are required for predetermination? 3. What is my out-of-pocket responsibility?
Provide your insurer with the documents gathered in step 1 and any other documents required
Step 4 - Book your appointment
Once coverage is determined, schedule your tattooing session
Payment is due in-full at your appointment
After your appointment, we will provide a superbill for you to submit to your insurer for reimbursement
